Boundary Blindness
in Insurance.

Insurance has spent thirty years digitising documents and transactions — but not decisions. The decision trail behind a risk or a claim (who reviewed what, under whose authority, against which rules, reaching what conclusion) still lives in emails, local systems, and the memories of people who are retiring. This paper calls the industry's inability to see that gap boundary blindness, and documents it across nine operational symptoms, sixty-one senior-practitioner interviews, and twenty-one independently published sources — each naming a piece of the same failure without naming the whole.

The cost was absorbable for three decades because institutional memory carried it. Three forces are now removing that absorber at once: a workforce ageing out, regulators demanding evidence of intent rather than action, and AI deciding at machine speed across boundaries it was never built to cross. The paper proposes a portable, verifiable decision record — the acceptance signal — and invites three platforms to test it in Phase Four. Early findings, honestly bounded; full findings published Q1 2027.

The Diagnostic

A structural condition the industry has not yet named: decision evidence routinely fails to travel across firm boundaries in a form the receiving party can rely on. The paper sets out the condition, why it has been operationally tolerable for three decades, and why three concurrent forces are removing that tolerance now.

Nine Operational Symptoms

Catalogued across cross-firm coordination, within-firm friction, and the AI-decision dimension. Each symptom documented in senior-practitioner interviews and corroborated against published institutional voices from regulators, reinsurers, trade bodies, and platform vendors.

The Convergence Cluster

Twenty-one independently published sources spanning six jurisdictions and seven sectors — from APRA to Munich Re to the Geneva Association to the NAIC — naming variants of the same architectural failure. The temporal density of recent publications is itself a structural finding.

The Architectural Implication

The category proposed by the paper is Portable Trust Infrastructure — coordination infrastructure that produces signed, portable decision records travelling across firm, platform, and jurisdictional boundaries without touching the underlying data. Phase Four tests whether it integrates in real platform environments.

From diagnosis to empirical validation.

Phase Four is the empirical step that distinguishes a working diagnosis from a tested one. Three software-platform participants engage in a structured twelve-week research collaboration to test whether the architectural intervention proposed in the paper integrates with low friction in real platform environments, and whether the practitioner-validated symptoms resonate with their own customer experience.

The broader programme welcomes participation from trade associations, standards bodies, research foundations, and individual practitioner experts as named partners, sponsors, or contributors to post-Phase Four standards governance.
